Old patient came to emergency unit in zagazig university hospital with facial trauma in a car accident.
After general examination and checking his vital signs and examination of neck vertebra, we examined his face.
We found facial odema and bruises, he had no teeth, and we found a step in his mandible from external palpation, the patient open his mouth and there was a malocclusion, his mandible and maxilla didn't close together. There were pain and tenderness on palpation.
The patient referred to radiology unit for radiological investigation. Computed tomography was done in axial, coronal, and 3 D maxillofacial cuts. The CT revealed bilateral parasymphyseal mandibular fracture with severe displacment in an edentulous patient with fragile mandible.
The patient was prepared for operation, laboratory investigation was done for assessment of patient's general condition.
Operation was performed under general anesthesia with nasal intubation. Sublabial approach was our approach to the fractured segment of the mandible. Before the incision, we infiltrate the wound with local anesthesia and decongestion ( adrenaline 1: 200,000 and lidocaine 2% ). This infiltration facilitate dissection and decrease bleeding during the operation and give a good postoperative analgesia.
Reduction of the cental displaced segment was done, and fixated by titanium plates and screws of mini-type. There were no teeth , so no intra-maxillary fixation or arch bar was done.
Patient successfuly extubated and sit for postoperative observation for one day and left the hospital in a good general condition.
Patient was instructed to avoid any chew for 2 month until the mandibule heal appropriately. All soft drink and food could be taken.
مريض مسن ليس له أسنان جاء الى الطوارئ بمستشفى جامعة الزقازيق بعد حادثة طريق. تم فحص المريض بالكامل للتأكد من عدم وجود اصابات أخرى بالجسد. وتم الاطمئنان على علامات المريض الحيوية وان حالته مستقرة
بعد فحص وجه المريض وجد ان هناك كسر وتورم بمنطقة الفك السفلي والمريض غير قادر على غلق الفم
تم عمل الأشعة المقطعية اللازمة على عظام الوجه والفكين ، ووجد ان هناك كسرين بعظمة الفك السفلي أدت الى انفصال الجزء الاوسط من عظام الفك السفلي. تم عمل مقاطع متعددة للكسر وهي مقاطع كورونال وأكسيال وثلاثية الابعاد
تم تحضير المريض لاجراء عملية اصلاح لكسر الفك السفلي تحت مخدر كلي حيث تم فتح مكان الكسر من داخل الفم من تحت الشفة السفلى والوصول الى عظمة الفك السفلي المكسورة و رد الكسر وتثبيته عن طريق الشرايح والمسامير من نوع التيتانيوم مقاس ميني
تم افاقة المريض بسلام وبقى تحت الملاحظة لمدة يوم بعد العملية للإطمئنان وخرج من المستشفى في اليوم التالي
